Sulforaphane (Broccoli Sprouts): The Best-Evidenced Dietary Cancer Prevention Compound

Sulforaphane is a compound found in cruciferous vegetables, with broccoli sprouts containing 20-100x more than mature broccoli. It activates the Nrf2 detoxification pathway, the body's master switch for producing antioxidant and detoxifying enzymes. Epidemiological data consistently shows that people who eat more cruciferous vegetables have lower cancer rates. A Phase II trial in men with recurrent prostate cancer showed PSA doubling time was significantly lengthened (slowing cancer progression), though the primary endpoint of 50% PSA decline was not met. Sulforaphane has the strongest prevention case of any dietary compound we've reviewed.
How It Works
- Nrf2 activation: Sulforaphane activates the Nrf2/KEAP1 pathway, upregulating Phase II detoxification enzymes (glutathione S-transferases, NAD(P)H quinone oxidoreductase, heme oxygenase-1). These enzymes neutralize carcinogens before they damage DNA.
- HDAC inhibition: Inhibits histone deacetylases, epigenetically reactivating tumor suppressor genes that cancer silences
- NF-κB suppression: Reduces chronic inflammation signaling that promotes cancer development
- Apoptosis induction: Triggers cancer cell death through mitochondrial and death receptor pathways
- Cell cycle arrest: Blocks cancer cell division at multiple checkpoints
- Anti-angiogenic: Inhibits new blood vessel formation in tumor models
Human Clinical Evidence
Phase II Prostate Cancer (Alumkal et al., 2015)
- Design: 20 men with recurrent prostate cancer, 200 μmoles/day sulforaphane-rich broccoli sprout extract for 20 weeks
- Primary endpoint: ≥50% PSA decline. Only 1 of 20 patients achieved this (not met)
- Secondary finding: PSA doubling time significantly lengthened in several patients, suggesting slowed progression
- Significance: The primary endpoint was aggressive. The PSA doubling time signal suggests real biological activity but at insufficient potency for advanced disease.
Epidemiological Evidence
The population-level data for cruciferous vegetables is among the strongest in cancer prevention nutrition:
- Meta-analyses show 15-20% reduced risk of colorectal, lung, breast, and prostate cancers with high cruciferous vegetable intake
- The Shanghai Women's Health Study and European Prospective Investigation into Cancer (EPIC) both found significant inverse associations
- Genetic studies show that individuals who metabolize sulforaphane slowly (GSTM1 null genotype) get less benefit, supporting a causal mechanism
Biomarker Studies
- A randomized crossover trial showed broccoli sprout consumption significantly increased Phase II detoxification enzyme levels in humans
- Studies in Qidong, China (high aflatoxin exposure area) showed sulforaphane-rich beverages significantly increased urinary excretion of aflatoxin-DNA adducts, confirming the detoxification mechanism works in humans
Why Broccoli Sprouts, Not Broccoli
Mature broccoli contains glucoraphanin (the precursor). When you chew it, the enzyme myrosinase converts glucoraphanin to sulforaphane. But:
- Broccoli sprouts contain 20-100x more glucoraphanin than mature broccoli per gram
- Cooking destroys myrosinase (raw or lightly steamed is better)
- Supplements vary wildly in sulforaphane content. Many contain glucoraphanin but lack myrosinase, meaning minimal sulforaphane is produced.
- Best approach: Grow your own broccoli sprouts (easy, $5-10 for a sprouting kit, harvest in 3-5 days) or use supplements that contain both glucoraphanin AND myrosinase (Avmacol, BrocElite are two that have been tested)
Practical Protocol
- Food source: 1-2 oz (30-60g) fresh broccoli sprouts daily, eaten raw or very lightly steamed
- Supplement dose: ~50-100mg sulforaphane per day (or 400-800mg glucoraphanin with myrosinase)
- Growing sprouts: Soak broccoli seeds 8 hours, rinse twice daily for 3-5 days. Cost: ~$0.50/serving
- Enhancement tip: Adding mustard seed powder to cooked broccoli provides exogenous myrosinase, restoring sulforaphane conversion
Safety
Sulforaphane from food sources is extremely safe. It's literally broccoli. Supplemental forms:
- GI discomfort at high doses (bloating, gas)
- Thyroid concerns are theoretical (goitrogens in cruciferous vegetables) but not clinically significant at normal intake
- No drug interactions of clinical significance known
- Safe during chemotherapy (some evidence it may enhance chemo efficacy and reduce side effects)
Our Assessment
Sulforaphane has the best risk-to-benefit ratio of any prevention compound we've reviewed. The mechanism is well-characterized and confirmed in human biomarker studies. The epidemiological data is strong and consistent. The Phase II prostate trial showed a signal. And the "supplement" is literally sprouted broccoli seeds that cost almost nothing. For cancer prevention (not treatment), sulforaphane from broccoli sprouts should be a foundational element. It's free, safe, and the evidence is as good as it gets for a dietary intervention.
